Release of Information PIN

Question:

How do I reset my Release of Information PIN?

Answer:

The Registrar Forms website includes the Release of Information form.

Guidelines:

  • Indicate specific records to release.
  • Designees name, relationship to the student, & their date of birth.
  • PIN - 5 digit.
  • Optional expiration date of release, otherwise the release does not expire without submitting a request to cancel.

How to Authorize Third Party Release of Information Over the Phone:

Designees must be able to provide their full name, relationship, DOB, and PIN. If they cannot, no account information will be released to them. If a designee forgets their PIN, they must reach out to the student.

If a student forgets their PIN, have them do one of the following:

  • Direct them to look up the history of all submitted Dynamic Forms by logging into myLCC.
  • Come into the StarZone or any off-campus service area with a picture ID.
  • Email Registrars Office from their LCC Student Email with a copy of their picture ID.

How to Authorize Third Party Release of Information in Person:

Designee must provide a valid state ID, driver’s license, or passport.

 

If designee does not have any form of identification, designees must be able to provide their full name, relationship, DOB, and PIN. If they cannot, no type of account information will be released to them.

 

Please Note: Previous Release of Information forms submitted prior to January 7th, 2019, are no longer valid. Therefore, any student who submitted a release prior to January 7, 2019, will need to complete the new release of information form located on the website under Release of Information. In accordance with The Family Rights and Privacy Act (FERPA) of 1974, LCC cannot discuss your academic and­­/or financial information with your parents, spouse, or guardian. By completing this dynamic form, you authorize LCC to discuss this information with your designee. Note: This form can also be used to add, remove, or change your designee(s) on your Release of Information.
